Seed Laboratory Sample Submission Form Instructions
Most results will be provided within 14 working days after the sample has been admitted for processing. You will be notified if processing time exceeds 14 working days. An invoice for services will be issued and sent to the billing address provided after the results are obtained.
Complete a separate form for each lot/species to be tested. Attach the form to each container of corresponding seed when submitting multiple samples at once.
Request forms will be processed upon receipt and tests will be completed on a “first come, first serve” basis.

Sender Information:
- Seed Company: Name of organization submitting sample.
- Contact’s Name: Name of individual responsible for sample submission.
- Billing Address: Address where invoice for services will be sent.
- Mailing Address: Address where report of analysis may be sent.
- Email: Most appropriate contact email for sending report of analysis.
- Office/Cell/Fax #: Most appropriate phone number to reach sender.
Sample Information:
- Genus/Species: Commonly accepted scientific name for seed submitted.
- Varietal Name: Indicate variety name here. If not known, indicate VNS.
- Common Name: Most accepted common name of species submitted. If mixture, indicate mixture here and provide a list of included species.
- Treated: If the seed is treated, select the yes box. Chemical: List the names of each active chemical ingredient.
- Lot Number: Unique lot ID associated with total seed amount.
- Total Pounds in lot: Total amount of seed present within lot.
- Total Amount of Seed Provided: Exact amount of seed provided within sample.
- Class Designation: If seed is certified, indicate class category. If uncertain, select service. Other is reserved for regulatory/research samples.
Tests Requested:
- Select all applicable test(s) that you would like performed per lot. Turnaround time will vary based on test category and species to be tested.
