Restricted-Use Pesticide Record Keeping
The following details for all restricted-use pesticide applications must be kept by the certified applicator for a period of two years. Records may be kept in any form (notebook, computer, state record form (forms below, etc.)
 
  • date of treatment
  • full name (first & last) of person applying or supervising
  • address/location of property treated
  • brand name or generic name of pesticide
  • EPA registration number
  • time the treatment was started and finished*
  • item or site treated
  • number of units, acres or volume treated or "spot treatment" *
  • purpose for which pesticide was applied *
  • total undiluted material and concentration or amount of undiluted material used (pints, ounces)
  • temperature start and finish *
  • wind velocity at the start & finish *

* = Required in the commercial categories of agricultural pest control-plant, chemigation, greenhouse and nursery pest control, forest pest control, fumigation, ornamental and turf pest control, right-of-way pest control or mosquito control; or in the private categories of agricultural pest control-plant, chemigation, greenhouse and nursery pest control, forest pest control, fumigation or ornamental and turf pest control.

Restricted-use records of state certified private applicators (individuals who produce an agricultural commodity) are inspected approximately once every four years. Applicators are selected randomly and notified by certified mail of upcoming inspections.
